What it usually means
Fog between panes points to a failed sealed unit.
When haze or moisture is inside the glass, it is not surface dirt. The insulated glass unit has likely lost its seal, so the practical repair is usually replacing that glass unit while keeping the existing frame if it is still sound.

Glass only or full window?
Keep the frame when the frame is still doing its job.
Foggy glass does not automatically mean the whole window needs to come out. The right scope depends on what else is happening around the opening.
Glass-only replacement may fit when:
- The frame is solid and not rotting, warped or leaking air badly.
- The sash operates normally and the hardware still works.
- The problem is limited to moisture, haze or staining between panes.
Full window replacement may fit when:
- The frame or sash is failing, draughty or difficult to operate.
- You want to change the window style, operation or configuration.
- Several units are aging and the project is part of a larger upgrade.

Can foggy double-pane glass be repaired without replacing it?
When moisture is trapped between panes, cleaning the surface will not remove it. The sealed glass unit has usually failed, so replacement of the insulated glass unit is normally the practical repair if the frame is still good.
Do you need to replace the whole window?
Not always. If the frame, sash and hardware are in good condition, glass-only replacement may solve the failed-seal problem. If the window system is also failing, full replacement may be the better scope.
